***Office Visits***

$450 New Patient Registration Fee. Includes a history and physical
exam up to 2 hours in length; also includes basic lab as determined by the physician.
(Fee due by credit card before you schedule your first appointment, and nonrefundable 
if you do not make your first appointment or cancel with less than one business day's notice);
This fee also applies to previous patients who have not been seen in the office for over 2 years.

$145 Office follow up visit, up to 30 min.

$275 Office follow up visit, up to one hour.

***Miscellaneous***

$45 each prescription prior authorization attempt from insurance, good for 6 months

$25 Insurance billing form replacement: if you would like another copy of your insurance reimbursement form
(the first one is provided at no charge if you request it at your office visit)

$45 each attempt to get a specialist referral authorization for you
from your insurance company (usually good for one year)

$45 for existing prescription call-in to pharmacy, or non-controlled "mail-in" written 
outside of an appointment, up to 3 meds per request

$45 convenience fee for lab ordered outside of an office visit (if you prefer)

$30 returned check or credit card chargeback fee 

$100 Rescheduling or missing regular appointment with less than one full business day notice 

$50 Drop-in fee (if you want to be seen in the office without an appointment);
Based on availability

$85 composing a letter or filling forms, each attempt, up to 4 pages 

$125 prescription pick-up fee; this is at your option, 
if you would like to pick up any routine refill, for a stable dosage that normally requires a visit;
only when medically appropriate, and only if you have been seen in the last 3 months; if you happen
to be out of town, we can send it via USPS Priority Mail with delivery confirmation for an additional $25. 
Please allow 7-10 days for delivery.

$100 fee for missed aesthetic procedure appointments or cardiac stress tests
cancelled with less than TWO business days notice.

***Virtual Visits***

$75  Phone "Virtual Visit" via Medical Assistant
$125 Phone "Virtual Visit" with Physician
(phone consult described below, up to 15 min)

*What is a "Phone" or "Virtual Visit" consult?

"Phone Visits" are pre-scheduled, live telephone consultations
with your provider.  These appointments last up to fifteen minutes
and may be used in lieu of a traditional Office Visit 
in appropriate situations. 

Certain conditions, such as colds, bladder infections,  
and management of chronic diseases (e.g. hypertension, diabetes, 
cholesterol) can be safely and appropriately managed in this 
setting.  Additionally, this is an option for discussing test 
results, medication changes, etc.  This is an alternative to 
coming to the office to see the provider, and saves you the time 
and cost associated with a traditional office visit.  This is 
appropriate for patients that have previously been seen in the 
office by the provider; only established patients may utilize this 
option. (This office does NOT practice telemedicine or Internet prescribing)

***Administrative charge payments:

At this time, 3rd party insurance payors do not adequately 
cover the cost of the above fees, so by using our services you agree that 
we will not bill it to them. Payment is due in advance, at time of request; 
a credit card is required. Our office is "non-participating" in Medicare/Medicaid.
