TexasMD was created to practice medicine differently.

Dr. Cyrus Peikari limits the number of patients in the practice so that visits can be longer, histories can be explored in detail, laboratory results can be reviewed carefully, and patients can have direct access to the physician when questions arise.

Why the practice changed

Dr. Peikari learned what “assembly-line medicine” could not do.

Earlier in his career, Dr. Peikari worked in a conventional high-volume medical environment and at one point served as Chief of Medicine. Like many physicians, he was trained to move quickly, diagnose efficiently, and treat the immediate problem.

Over time, a pattern became difficult to ignore: patients could improve in the short term while the larger problem remained unresolved. A brief visit often left too little time to understand the full history, the failed treatments, the subtle symptoms, the patient's priorities, and the relationship between all of them.

That experience changed the way Dr. Peikari practices medicine. TexasMD now emphasizes longer visits, careful listening, extensive review, and decisions made with the patient rather than around the patient.

The goal is not simply to add another prescription. The goal is to understand what is actually happening, decide what deserves attention, and build a practical plan that the patient can follow.

About the physician

Cyrus Peikari, M.D.

A Dallas physician with deep local roots and a career that has included medicine, engineering, teaching, writing, ethics, public speaking, and physician leadership.

Dr. Peikari has lived in the Park Cities / Preston Hollow area since 1969. He earned a Bachelor of Science with honors in electrical engineering from Southern Methodist University, worked in telecommunications and software engineering, and then earned his M.D. from Southwestern Medical School in Dallas.

His medical training included an honors rotation in Urology, a clinical externship in Neurology at Oxford Medical School in England, and Internal Medicine training at Presbyterian Hospital of Dallas.

He has authored multiple books, including a textbook used by physicians studying for their board examinations. He has also taught professional ethics courses for the Texas Medical Association, has conducted radio and television interviews, has received multiple U.S. patents, and has served as Chief of Medicine at Baylor Medical Center at Garland.

Despite those accomplishments, the center of the practice remains deliberately simple: helping patients one at a time.

The first visit

More time to listen. More time to understand.

The first appointment is designed to give the physician and patient enough time to understand the problem before deciding what to do about it.

1. An extensive history

Dr. Peikari reviews the patient's medical history in detail, including symptoms, previous diagnoses, medications, treatments, goals, and—importantly—what has not worked.

2. Physician examination

The initial evaluation includes a physical examination performed directly with the physician.

3. Laboratory evaluation

Extensive laboratory testing may be obtained when appropriate. The current TexasMD program includes applicable first-visit laboratory testing with the first-visit fee.

4. A detailed follow-up

When laboratory results are available, the patient returns to review them in detail, ask remaining questions, and discuss the recommended next steps.

5. A plan chosen together

The resulting plan may involve lifestyle changes, nutrition, medication when medically appropriate, and an ongoing membership level based on the frequency and complexity of care needed.

A remarkable guarantee

The first visit comes with a full money-back guarantee.

If a new patient reaches the end of the first visit and is not completely confident that TexasMD is the right clinic, the first-visit fee may be refunded under the practice's stated guarantee terms.

There is no need to invent a reason or defend the decision. The purpose of the guarantee is simple: a patient should not feel trapped into continuing with a medical practice that does not feel like the right fit.

TexasMD reports a 97% satisfaction rate and says that nearly all of its clients arrive through word-of-mouth referrals from existing patients.

The refund must be requested on the same date as the first visit, before leaving the office. Refund processing time may vary by bank. Full current terms are available from the office.

Direct access is the product—not five-minute appointments.

Once a patient becomes a member, TexasMD provides access to Dr. Peikari by phone, text, email, and in person. The practice deliberately limits enrollment so that this level of access can be maintained.

Direct physician access

Members can reach the physician by phone, text, email, and in-person visits rather than routing every question through a large system.

Care matched to need

Membership level is based on the frequency, intensity, and complexity of care required rather than a one-size-fits-all visit model.

Outside the insurance system

TexasMD has opted out of third-party payors, including private insurance, Medicaid, and Medicare, in order to preserve its direct-care model.

Why act now?

TexasMD intentionally does not try to see everyone.

The practice limits the number of new patients so existing members can continue receiving the time and access they were promised. An appropriate prospective patient may therefore encounter a waiting list.

For someone who has been postponing a health concern, repeatedly leaving appointments with unanswered questions, or simply wants a more personal relationship with a physician, another six months of waiting is unlikely to create more clarity.
